Speaker or Meeting Request Forms
The forms below are necessary for requesting a speaker from Truth Ministry for any event or to request a meeting of a non-counseling type. This would be for pastors, counselors, social workers, or students. They help us to know your needs and fully inform you of the process involved. The forms ask needed information and provide necessary information.
On the bottom of the form, it will ask you whom you would like to meet with or have come speak. If you do not select one, then the ministry leader best suited or closest to you will be selected for you. After you complete your form, it will be forwarded to the appropriate leader, and they will be in touch with you to set up for your event or appointment, if one has not already been scheduled.
A valid email address is necessary to complete an online form. Setting up an email account in Yahoo or another provider is a very easy process and usually free.
Confirmation and Problems with submitting form, etc.: When you submit the intake form, you will receive an onscreen thank you and an email with the information you have submitted. If this does not happen, the intake was not completed properly for some reason. If you do not complete the form thoroughly, it will not let you submit the form.
