Prospective New Clients - Intake Forms
If you know now that you would like help for any form of counsel or support group involvement, other than a brief phone or email conversation, we will need to get initial intake forms completed by you. These must be completed prior to any ministry involvement or in depth help, including telephone or email. After reading this info, see intakes at bottom of this page.
A valid email address is necessary to complete an online form. Setting up an email account in Yahoo or another provider is a very easy process and usually free.
Security of online forms: Truth's website is secure and all information is encrypted and verified through Network Solutions.
Confirmation and Problems with submitting form, etc.: When you submit the intake form, you will receive an onscreen thank you and an email with the information you have submitted. If this does not happen, the intake was not completed properly for some reason. If you do not complete the form thoroughly, it will not let you submit the form. To prevent the loss of your information, follow the instructions above the submit button. If you encounter another type of error code, turn off your fire wall and turn off your pop-up blocker. Text fields and Text boxes: Text fields will hold up to 255 characters, including spaces. Text boxes will hold 60,000 characters, including spaces. This is the equivalent to 3 to 4 pages. If you have a lot of information that you would like to share with us, we recommend you write it in Word and measure it. If it exceeds these character allowances, then send it in a separate email to us as an attachment.
Exceptions to the Online form process: We can go through the intake forms with you in person, if that would make you feel more comfortable. This adds at least one to two counseling appointments (suggested donation rates apply). Please contact us if questions or un comfortability cause you pause about the online process. If someone does not have access to the internet or a library, due to a physical handicap, then we will be willing to mail a paper copy of the form(s) out. You may request a printed copy of the form(s), if you are unable to complete them online or through email. A leader will then have to transpose your hand written answers into the form, so we ask that you not utilize this option unless necessary. |
